近五年內主要研究內容敘述

一、動脈壓變異性分析與應用 (呼吸影響波動,BHF;血管運動影響波動,BLF)

  BHF:動脈壓呼吸波動與自律神經功能之關係,長久以來一直為學者所忽視。本研究室1999年論文1 首次證明動脈壓呼吸相關律動可做為評估大鼠之心臟交感神經活性。另兩篇論文更詳細探討此頻率成份來源及其機轉2, 3。另外,動物失血時會引發此波動功率之上升具專一性,研究結果發現失血所引發之現象與自律神經之β-交感神經活性具極密切之相關4;由於動脈壓訊號於醫院中容易取得,故於人類研究亦證明此波動與β-交感神經活性有關5,將來更有非侵體性測量技術發展之可能。故此部分論文可提供一項發展潛力極高之心臟交感神經功能或失血現象評估方法。

  BLF:本研究室曾參與動脈壓低頻律動之臨床應用及其來源之探討,已知其與交感神經對血管調控有關。高血壓疾病一直威脅人類健康且名列十大死因之一,然至今對它的瞭解仍然不多。事實上血壓的調節乃一動態之平衡,綜觀多數研究皆以靜態方式探討,恐失去許多重要訊息。吾人嘗試以動態角度觀察SHR與正常血壓對照鼠 (WKY) 之差異並研究其延腦動態控壓能力 (或謂血管對神經之反應性) 之差異,吾人首度證明SHR的延腦動態控壓能力強於WKY鼠,並有較快之反應速率,更詳細洞悉高血壓形成原因6。並進一步探討交感神經於其間扮演之角色。另本研究室成功開發自由活動大鼠之動脈壓變異性分析研究模式7,於此動物模式亦得到類似之研究結果與結論。但此現象僅獨特出現於睡眠時期,更指出睡眠研究之重要性,詳見第三部份的敘述。

  二、心率變異性 (HRV) 之分析與應用

  由於訊號取得較易,心率變異性之研究開發較早。本研究室以自製之心電放大器 (郭教授設計) 及配合撰寫分析程式 (郭教授撰寫),與公衛專家合作將記錄到的台灣南投縣漁池鄉中老年等數千人之資料作分析,完整評估健康人心臟自律神經之功能參數,發現五十歲前男性在休息狀態下之基礎交感神經活性比女性高;相反的,女性之基礎副交感神經活性則比男性高。但此現象於五十歲後男女差別即消失8,推論此現象與女性五十歲左右時停經現象有關;其後與地區衛生所合作,更針對此問題探索同年齡層五十歲左右之男、女自律神經活性差異之謎。確認女性內生性性激素於停經前扮演重要之角色。女性停經後若補充雌激素則可回復至停經前之自律神經功能表現9;之後以自由活動大鼠之研究模式,探討內生性雌激素之角色,也確認雌激素其促進副交感基礎活性之作用10;此方面研究也與傳統醫學醫師合作,發現針灸對停經婦女之自律神經失調具改善效果。有趣的是,針灸亦能一併改善停經後之睡眠問題11;應用到婦產科子顛前症懷孕病人,發現病人有交感活性過度興奮而副交感活性異常降低現象,提供臨床治療一重要訊息12;針炙應用之研究也發現,由於人面臨壓力或各種病徵時,往往增強交感而抑制副交感神經活性;而針刺四神囪穴可有效加強副交感且抑制交感神經活性,可提供臨床治療上方便且少傷害之重要方法13。與腸胃科合作開發評估人類及大鼠,胃之自律神經功能的新方法14-16;與神經內科確認中風對自律神經功能的影響17 神經外科探索癲癇病人的自律神經功能變化 18-20;與泌尿科探索心臟自律神經系統於男性性功能障礙之角色21;與精神科及藥師探索人類及大鼠之精神疾病、用藥情況及治療對心臟自律神經功能之影響22-31;與精神科探索實習醫師之壓力問題32-34。另也瞭解美沙冬對毒物成癮的替代療法對心臟自律神經功能之影響26, 28。另人格特質與自律神經功能也有一定關係35,36。另外,利用控制呼吸頻率與吸氣及呼氣時間比例,來探究各項HRV參數及感壓反射靈敏度的變化及影響,詳細了解HRV及感壓反射靈敏度設計實驗時,呼吸影響的重要性37-40

三、人與大鼠之睡醒與自律神經相關研究

吾人現擔任陽明大學睡眠研究中心主任,積極與基礎與臨床研究者合作,利用過去已確立的人及自由活動大鼠之研究模式41, 42,而成為本研究室近年的研究主軸。過去本研究室可量測到人及大鼠之心電訊號,並詳細觀察睡眠中自律神經之變化43, 44,也詳細探討其中自律神經功能與睡醒之因果關係45,並進一步解答自律神經功能於睡及醒調控之正常生理及可能病理機轉。研究發現有 (1) 傳統醫學合作發現停經婦女的睡眠問題與自律神經調控實具高度關連,並可透過針灸適度緩解自律神經之異常,亦能有效改變睡眠品質11(2) 護理人員值班 (長期值夜或輪班) 的睡眠自律神經功能變化46-48,提醒值班設計的重要性。(3) 探討SHR之睡眠與自律神經功能變化,SHR睡眠結構與WKY具明顯差異性,並具明顯睡眠片斷現像49-51,另特別於睡眠時才出現循環自律神經調控缺陷51Circulation期刊還特別為我們的研究設計及重要發現寫評論 52。也找出SHR之睡眠異常可能與交感神經1-adrenergic system過度興奮有關53。近年我們更建立完全的無線大鼠模式,除在測量時動物能更自由、更無壓力外,亦使記錄時間可以延長並有機會 (4) 瞭解超過24小時之正常生理律動54及改變光照造成之影響 (結果整理中),可以詳細在控制環境下探討所謂時差等因素對人的可能影響;(5) 本實驗室更自製遙控電刺激模式,電興奮血管運動中樞 (rostro ventral lateral medulla, RVLM),證明正常血壓鼠可因RVLM之刺激使睡眠模式轉變為類似SHR之片斷式睡眠。上述之研究結果可間接解答SHR可能是因為RVLM 過度興奮造成睡眠片斷現像55,電刺激RVLM造成醒覺的發現,期刊也特別為我們的研究及發現寫評論 56 (6) 利用大鼠模式,仔細了解人類一日於夜間睡眠時血壓下降 (blood pressure dipping, BP dipping) 在正常生理之角色,其可能之機轉。確認其中醒睡於BP dipping之比重佔超過90%,而日夜節律則僅佔不到10% 的影響,進一步探討BP nondipping之可能病理原因57;另,已知睡眠後期是全天中最高好發心循環疾病的時間,我們也確立了與人類相似之大鼠研究模式,了解此期間可能與睡眠後期之睡眠結構不穩定及自律神經系統偏向交感神經活性有關53(7) 另,利用人類及大鼠之動物模式分別進行研究冬季好發心循環疾病的可能機轉,實驗設計特別為台灣冬季低溫模式,且已確認許多可能機轉58(8) 除探索好發心循環疾病的問題,我們也試著了解運動降血壓之機轉,希望提出更好的預防及治療高血壓模式 (大鼠及人都已有結果,一篇投稿中,一篇撰寫中)。目前正積極探討上述各因子於高血壓之特異表現。(9) 小鼠的研究模式已完成研究技術之建立,目前論文正投稿中。此方面更開啟與分子生物領域研究合作之可能。(10) 另我們也發現自覺入睡困難者其白天副交感神經活性較弱及交感神經活性偏高,入睡過程中腦波高頻波活性也偏高於睡眠品質良好者,顯示入睡困難問題與皮質過度興奮有關;而慢速 (0.1 Hz) 定量呼吸訓練,可有效降心率及調升整體自律神經功能,同時並能有效改善客觀睡眠品質,特別是縮短入睡時間以及提升睡眠效度59,本論文也提出入睡困難失眠者有效的非藥物治療方法,此議題也將是本實驗室未來探索睡眠問題的主軸。(11) 我們與床枕公司合作探討以躺床之肌力測試來選床,發現短期間睡在選出較適的床上,可有效改善循環神經調控及感壓反射靈敏度60,突顯睡眠時床墊選擇的重要性。

除了睡眠研究,我們也將視野跨到人非睡眠時期之白天自律神經功能變化,並看午睡對自律神經功能的影響及其可能生理意義 (論文投稿中)。近年更將研究技術方向拓展到雲端人性化之睡眠 (以研究室自行開發之活動計量測) 及心循環系統等功能測量,使睡眠及各式生理參數的記錄更生活化更普及化。不用等到研究成果出來,受試者只要參與監測生理訊號即對自己的身體保健就能多一層了解及關心,軟硬體架構幾乎完成,目前正積極進行各式人體試驗。

四、正在運動中大鼠之腦波與自律神經研究

  因本研究室近幾年已建立自由活動大鼠模式之腦波、肌電波、血壓及心電波等多訊號記錄,我們將探索範圍延伸至活動狀態下,特別探索正在運動中大鼠的腦波、肌電波及心電波。我們已成功記錄大鼠進行等速跑步機運動時的腦波及心電訊號,發現在運動過程中大腦枕葉theta頻帶成份會增加,而已知此頻帶可能與認知能力有關61, 62。由於這是之前許多運動研究上較少能做到的記錄及各式控制,所以在這方面研究上仍擁有許多優勢。並記錄大鼠海馬迴之腦波發現,跑步運動可依照theta波成分作分期,並藉此探討感覺運動整合的過程。另,亦記錄自由活動之滾輪運動大鼠,探討速度與theta波之關係63。最後藉由兩種運動模式探討自願運動被動運動之差異,發現兩種運動雖速度及活動力都不同。經過分析比較後,我們認為theta波的高頻成分可能與心理壓力有關64。此外我們也進一步探討老化對運動過程中腦波的影響,發現老化會減低運動對腦波的反應,此現象僅於運動中才觀察的到65。再者,我們亦觀察老化對運動過程中大鼠心率變異性之影響,結果暗示老化為造成心臟調控能力簡化之因素之一,是值得注意的,此部份正投稿中。另透過記錄在水管內移動之大鼠生理訊號,以釐清視覺及身體活動對theta波產生之貢獻66過去我們確認SHR的運動訓練確實可減緩隨年齡增加的高血壓及睡眠問題 (投稿中),今年擬更進一步了解SHR的運動訓練是否也可防止因血壓上升引發的學習記憶能力缺陷的問題,其中自律神經系統、睡眠及腦波成份等各生理參數是否亦扮演重要角色。
